Changing your appointment

The change appointment form is designed to allow you to change your existing appointment to be more convenient or you can use it to request an appointment (if you are over 70 and not included in the age extension pilot).

Change Your Existing Appointment
If you have received an appointment from our office but it is not convenient you can use this page to request an alternative location, date or time.

Request an Appointment (Over 70s)
If you are over 70, not included in the age extension pilot, and have not been screened for over 3 years you are also welcome to complete this form to request an appointment.
